Drug and Alcohol Rehab Treatment Planning
We are committed to developing multiple, well-defined mental illness
and drug and alcohol rehab treatment tracks for you, focusing on your specific
needs, to restore your health and well-being.
We do so by establishing a comprehensive psychological disorder and
drug or alcohol addiction treatment plan that
includes milestones and goals based on your personal challenges and
needs. The treatments and therapies you receive are tailored
specifically to you; your customized treatment plan is relevant to your
unique treatment goals.
This plan is designed to take you from entry to discharge and
beyond to ensure lasting recovery. Our multi-disciplinary team
reassesses this plan weekly, taking into consideration your progress,
preferences and any new developments that arise along your journey to
recovery. This personalized attention to your treatment means you get
the best possible path to wellness, bar none.
We encourage family and personal therapists and physicians to
participate in your psychological disorder or drug/alcohol rehab treatment planning.
If needed, we also offer video conferencing to facilitate long-distance meetings.
Case Management
After your initial assessment,
you will be assigned both a Counselor and a Therapist, who will provide
you with the individual support you need throughout your stay at New
Seasons drug and alcohol rehab center.. Their role is to help you understand your treatment goals,
coordinate your care, monitor your progress and liaise with other staff
members to ensure your needs are met and that you are on target to
achieve your goals.
Discharge Planning and Aftercare
We begin
planning your successful discharge as soon as you arrive. This ensures
that as we work toward your recovery, we also put into place the
support system you will need to return home. This might
include optional professional assistance such as specialized
physician/therapist services or support groups/resources in addition
to a Twelve Step sponsor and other relevant social support.
Upon discharge, you will be provided with a comprehensive
aftercare treatment plan that addresses your ongoing needs, including any
needs for: therapy, medication, nutrition, exercise, personal relapse
prevention plans, and community support. Once you decide what your next
step will be, your Case Managers help you manage this step by
coordinating your transfer and communicating all relevant information
to those who need it.
An optional service includes escorting you home to meet with
your family, physician, therapist and/or other aftercare support to
solidify a healthy lifestyle and maintenance plan.
